Returning students or Incoming freshmen from Memorial Junior High or Heritage Middle School
Book pick-up/Registration information
2019/2020 School Fees are $300 (some fees may be waived for students who are eligible for Free or Reduced price lunch)
- Textbook Rental $125.00 ($100 if paid during book pick-up)
- General and Curriculum $150.00
- Technology Fee $ 25.00 (fee cannot be waived)
For your convenience, acceptable forms of payment include cash, personal check, cashier’s check, money order, Visa or MasterCard. If you pay with a check, it should be made payable to T.F. South High School. A $25 service charge will be applied to all checks that are returned “Non-Sufficient Funds.” The remaining balance must be paid in cash, by money order or credit card by April 1st of the student’s senior year.
Until the debt has been paid in full, a student may graduate but will not be eligible to participate in prom, nor will a final transcript be issued.
VERIFICATION OF RESIDENCY
For those parents/guardians of RETURNING students who produce a valid driver’s license or state ID with a current District 215 address, no further documentation will be required.
SUBMITTAL OF REQUIRED MEDICAL/IMMUNIZATION DOCUMENTATION AT REGISTRATION
Physical Exams – Freshmen
All incoming freshmen and transfer students must submit a copy of a current physical and up-to-date immunization records before entering T.F. South.
The health history must be completed, signed and dated by the parent/guardian.
The physical examination portion must be filled out completely (including the immunization record, diabetes screening and approval for physical education), signed and dated by the examining physician.
The school entrance physical and the athletic physical are two different forms. The only form accepted for entrance into the ninth grade is the currently approved IDPH Certificate of Child Examination. Athletic physicals are not accepted for high school entry.
Illinois State School Code requires that all freshmen and transfer students submit a copy of a current physical and up-to-date immunization records before entering T.F. South. Students will not be allowed to complete the book pick-up process or be enrolled in District 215 without proof of health examination and immunizations. IMPORTANT NOTICE TO ALL PARENTS AND STUDENTS REGARDING MEDICAL REQUIREMENTS
In addition to the health exam and immunizations, Illinois Public Act 099-0843 requires schools to request an Asthma Action Plan for all students diagnosed with asthma. The Asthma Action Plan must be written by your child’s physician, kept on file in the School Nurses’ Office, and be updated annually.
If you currently have an Asthma Action Plan on file for your child, no further action is needed for this current school year. If your child has asthma and does not have a plan on file, please provide a plan to the school nurse immediately. District 215 maintains a Residency Hotline which may be contacted 24 hours a day. Parents and community members are encouraged to contact this hotline if they suspect someone is attending district schools as a non-resident. All hotline tips should include detailed information that will be investigated confidentially.
