Guardian In-Person Self-Certification Agreement


Welcome back to in-person learning for the remainder of the 2020-2021 school year!

In order to comply with Illinois Department of Public Health (IDPH) regulations, all parents/guardians of returning students must be certify that their students are free of known COVID-19 symptoms before entering school buildings. Students, as well as all district employees,  are also required to wear face coverings, as outlined in the attached Board Policy 4:182.

The District will not require the completion of a daily COVID‐19 Self‐Screening checklist for students; however, all parents/guardians will be required to digitally sign an Guardian In-Person Self-Certification Agreement, which will be kept on file and must be adhered to for the duration of the COVID-19 pandemic. This Agreement must be completed by all families of returning students prior to April 12, 2021 in order to enter the building. NOTE: It is recommended that you use Google Chrome rather than Microsoft Edge to complete this Agreement.
