When can I enroll my student?

When can I enroll my student?
Enrollment takes place on Mondays and Wednesdays between 8:00 a.m. and 11:00 a.m. 

Can anyone enroll a child?
No, only the parent or legal guardian can enroll a child. 

If I'm not the parent, what do I need to prove guardianship?
Court documentation signed by a judge, stating the reason from the change of custody. 

If the father is not on the birth certificate, can he enroll the student?
No. 

Are registration requirements different if I have a foster student?
Yes, a 906 form must be provided. 

What do I need to register/what documents do I need to bring for the last school attended?
All required residency documentation as indicated below.

 

  • Original Birth Certificate (if incoming Freshman)
  • Illinois State Transfer Form. If coming from an out of state or private school, a letter on school stationary stating student is in good standing or discipline record
  • Current physical with doctor's signature and immunization record (with a recent T-dap immunization if a freshman). Make sure that the doctor indicates that the student can participate in P.E. and Interscholastic Sports and that he/she signs and dates this portion [on back] of the form
  • Transcript. (If a freshman, you will not have a transcript till 2nd semester)
  • Grades in progress - if transferring after the 10th day of a semester
  • 8th grade diploma - if transferring as a freshman in August.
  • EXPLORE test results - if transferring as a freshman in August.
  • I.E.P (if applicable)

What if my student did not take the EXPLORE test?
During the summer contact Mrs. Taylor at (708) 585-2024 to make an appointment to take the test. Once school starts, please contact Mrs. Bradley at (708) 585-2015. 

What are acceptable proofs of residency?

What if I live with another household?
The homeowner must provide one (1) item from Category 1 and two (2) items from Category II.
The registrant must ALSO provide two (2) items from Category II. 

Will copies of the birth certificate be accepted?
No, you must provide an original birth certificate. A copy will be made and the original will be returned to you. 

Can a student be enrolled if registration is incomplete?
No, you must bring all the required documentation before registration will take place. 

Once the registration is complete, will my student start on the same day?
No. 

Should I apply for free or reduced lunch?
We encourage all families to complete the Free and Reduced Lunch application, which can be found on this website under forms. The Free and Reduced Lunch application is used for District 215 to qualify for federal grant money. 

If I qualified last year, do I need to reapply?
Yes, you must apply each year. 

When will the new free and reduced lunch form be available?
The 2011/2012 free and reduced lunch form will not be available until 8/1/2011. 

If I applied in another district, do I still need to apply at T. F. South?
Yes. 

Do I need to fill out a separate form for each one of my students?
No, unless a foster student lives in the household as identified with a 906 form on file. 

If I qualify for free or reduced lunch, will that impact my book fees?
Yes. 

When will I know if I qualify?
During mass book pickup. If you enroll after the beginning of the school year, you will be notified within 24 hours.
